Formidable Forms' use template button
Clicking the “Use Template” button will redirect you to the Formidable Forms builder.

You can access all the customization tools on the left side. On the right side is the live preview of the form.

We can notice that in this form template there are pre-set questions.

For add-ons, you can click on each field. Then, simply adjust the written copy from the customization ukraine telemarketing data panel on the left side.

For this guide, we will create a chained health and wellness quiz as an example.

In this case, let’s change the first question from “Which of these is unlike the others?” to “What is your primary fitness goal?”

We then check the 'Required' checkbox to ensure that users do not skip this question.

If you want, you can change the style of the radio button . We chose “Simple”, but you can use images or install the “Button” add-on to have access to other button styles.

At this point, you will need to scroll down to the custom panel and change the options for this question.

The options are “Weight Loss”, “Muscle Gain” and “General Health”. By default, the form template has four options. We really want to change this option too.

To remove an option, simply click the “-” button to the right of the option field.
